Kontos suffered a flexor strain and will be placed on the disabled list, Alex Pavlovic of the San Jose Mercury News reports.

Kontos has made eight appearances through the Giants first 14 games, giving up one run on five hits in 5.2 innings while mostly pitching in medium-leverage situations. Flexor strains typically sideline pitchers beyond the minimum 15 days, and the Giants' bullpen is already missing Sergio Romo, who recently suffered the same injury. The team figures to call up a reliever Tuesday.

Hernandez allowed two solo home runs in an inning of work during Monday's loss to the Mets.

The homers by Lucas Duda and Neil Walker broke a streak of six scoreless innings for Hernandez. He's had a couple of blowups already this season, leaving him with a 7.11 ERA, but Hernandez also has an 11:4 K:BB in 6.2 innings.

Lara threw two scoreless innings in Monday's loss to the Salt Lake Bees.

Lara has started the 2016 season strong, as he has thrown for a 1.29 ERA in five games while limiting opponents to a .130 batting average. However, it's far too early to assume that he's headed for the majors. Lara has a career 4.64 ERA in the minors and allowed 65 hits over just 50.1 innings across two levels last season.

Blach took a loss Monday against Triple-A Salt Lake after giving up five earned runs in three innings.

Blach's season has been very shaky over his three starts. In three games, he's gone 1-2 while posting a 6.75 ERA. Despite these rough performances, Blach is still at the top of Sacramento's pitching rotation, which is a promising sign for a player in only his second season of Triple-A baseball. Unless his poor outings persist and he's sent down to Double-A Richmond, Blach is likely to stay in Triple-A the rest of the season to keep developing his game.

Weber was recalled from Triple-A Gwinnett on Monday.

The right-hander is replacing Gordon Beckham (hamstring) on the 25-man roster after Beckham was placed on the 15-day disabled list. Weber pitched well in 8.1 innings in the minors this season, which warranted a call-up to the big leagues. Given how badly the injury bug has bitten the Braves bullpen so far this season, Weber could get a decent number of chances to pitch for Atlanta during his time in the big leagues.

Vizcaino has no runs allowed and one save in 4.0 innings over five appearances this season. He also has five strikeouts and three walks.

Owners would like to see that 1.50 WHIP drop a bit, but that's mostly due to allowing four base runners in his last appearance. Vizcaino is 1-for-1 in save opportunities and owns a healthy 11.25 K/9. He should continue to be the Braves' go-to guy in the ninth inning.

Jansen picked up his fifth save of the season Sunday against the Giants.

The right-hander allowed his first hit since April 5, and just the second hit he's allowed all season. Sunday's save marked the first appearance this season in which Jansen didn't record a strike out. Having averaged more than 33 saves a year over the past four seasons, Jansen is an elite option at closer in all formats.

Gomez picked up the win in the Phillies 3-2 victory over the Nationals on Sunday.

Gomez had pitched a scoreless ninth, but then gave up a solo home run to Bryce Harper in the top of the 10th. The Phillies scored two runs in the bottom of the 10th off of Jonathan Papelbon to bail Gomez out. He is 1-0 with four saves, a 2.57 ERA and a 6:2 K:BB in seven innings.

Archie Bradley and not De La Rosa will start Monday for the Diamondbacks, Nick Piecoro from the Arizona Republic reports.

De La Rosa pitched in relief Saturday night and served up Melvin Upton Jr.'s game-winning homer. His appearance was necessitated by Shelby Miller's truncated outing.

Barraclough was recalled from Triple-A New Orleans on Sunday, Barry Jackson of the Miami Herald reports.

The Marlins' bullpen as a whole has underperformed in the early stages of the season, so the team will make a minor shakeup by bringing aboard Barraclough and designating reliever Dustin McGowan for assignment. Barraclough has fared well during his brief stay at New Orleans this season, accruing a 1.50 ERA over six innings while posting nine strikeouts versus one walk.
