Melancon (elbow) will make his second rehab start with High-A San Jose on Wednesday, broadcaster Joe Ritzo reports.

Melancon was near perfect in his first rehab start with Triple-A Sacramento, tossing a scoreless inning while allowing a lone walk. The veteran reliever will likely need a couple more outings before the team determines his readiness for major-league action, but all signs have been positive over the past couple weeks, so it's logical to expect him back in the Giants' bullpen before long.

Clarke was promoted to Triple-A Reno on Tuesday, Kyle Glaser of Baseball America reports.

The 24-year-old Clarke earns the call to the minors' highest level after rolling to a 2.91 ERA and 1.20 WHIP to go with 107 strikeouts in 111.1 innings with Double-A Jackson. The right-hander is one of the better arms in a Diamondbacks farm system lacking in high-end pitching talent, but despite finding success throughout his three seasons in the minors, he doesn't project as much more than a No. 4 or 5 starter once he reaches the big leagues.

Nelson (9-5) gave up two runs on six hits and a walk while striking out seven over six innings in Tuesday's 3-2 win over the Cardinals.

It was the right-hander's first career win over St. Louis in 11 tries. Nelson has now delivered quality starts in five of his last seven outings, continuing his breakout campaign with a 3.09 ERA, 0.98 WHIP and dominant 56:8 K:BB in 43.2 innings over that stretch. The burgeoning ace will try to keep that momentum going in his next start Sunday in Tampa Bay.

Yates picked up his 10th hold after firing a scoreless inning in Tuesday's 3-0 win over the Phillies.

Padres closer Brad Hand was not dealt at the trade deadline, leaving Yates as the team's setup man. While this is disappointing news for those who were speculating on potential save opportunities, the dominant righty should continue to provide value as a ratio-boosting reliever (2.17 ERA, 0.96 WHIP and 13.5 K/9) in addition to accruing holds down the stretch.

Sims (0-1) gave up three runs on six hits over six innings while striking out three and walking none in Tuesday's 3-2 loss to the Dodgers.

The 2012 first-round pick didn't appear nervous at all in his MLB debut and kept the Braves in the game, throwing 62 of 92 pitches for strikes and displaying the improved control that earned him a promotion to the majors. Including his time with Triple-A Gwinnett, Sims has now given his team a quality start in five of his last seven trips to the mound, but he'll look for a better final result in his next outing Sunday at home against the Marlins.

Jansen struck out all three batters he faced while converting his 28th save in Tuesday's 3-2 win over the Braves.

It was truly a perfect inning for the dominant closer, but this has become the norm for Jansen, who has thrown five similar outings this year in which he struck out every batter faced while notching a save. The 29-year-old leads all closers with a 1.33 ERA while trailing only Greg Holland and Alex Colome in saves.

Hand struck out two in a perfect ninth Tuesday against Minnesota, recording his seventh save.

Widely rumored to be on the move ahead of the trade deadline, the veteran lefty has instead stuck around in San Diego, where he'll continue to serve as the closer; he's converted saves in his last five appearances, allowing just a single hit in that span. Fantasy owners who plucked him off the wire certainly won't complain -- why bite the Hand that feeds you?

Hoffman held the Mets to two runs on five hits over six innings at Coors Field on Tuesday, striking out five and walking three in a no-decision.

Having surrendered 21 earned runs with a 10:10 K:BB in 20 innings spanning his prior four starts, Hoffman wasn't likely a popular choice for Tuesday's fantasy lineups, but he delivered his finest performance since a late-June start against the Giants. Though he's still relatively young with upside, Coors isn't doing him any favors; however, he does have a 3.65 ERA and 1.03 WHIP on the road, suggesting he might have some selective streaming appeal in deeper leagues.

Collins (elbow) logged one scoreless inning in his debut appearance for High-A Potomac on Sunday.

Collins hadn't pitched in games since 2014 prior to his rehab assignment with the Nationals rookie-league affiliate, so the team will bring him along slowly as he gets acclimated to professional baseball again. He will likely find his way back to the upper levels of the minors later this season or in the early portion of 2018.

Bracho was optioned to Triple-A Reno on Tuesday.

Bracho pitched a scoreless inning after being recalled on July 29, but with infield depth the more pressing need at the moment, the reliever will head back to the minors. Jack Reinheimer will replace him on the active roster.
