Hernandez signed a one-year contract with the Phillies on Wednesday.

Hernandez elected free agency following a 2015 campaign in which he posted a 1-5 record with a 3.28 ERA and 1.31 WHIP over 33.2 innings with the Diamondbacks. The 30-year-old reliever will join a relatively weak Phillies bullpen, and should serve in a middle relief role for the club.

The Braves agreed to trade Miller to the Diamondbacks on Tuesday, pending a review of his medical records, Ken Rosenthal of FOX Sports reports.

Days after signing Zack Greinke to a six-year deal, the Diamondbacks have pulled off another stunner, acquiring Miller and his three years of team control from Atlanta. The return package for the Braves is said to include Ender Inciarte as well as top prospects Dansby Swanson and Aaron Blair. Miller's lone season with the Braves was a strong one, as he held opponents to a .238 average and finished with a 3.02 ERA over 205.1 innings. He should benefit from more run support in Arizona after earning just six wins in 2015.

GM Matt Klentak confirmed Monday that there has been a lot of interest from other teams in trading for Giles, CSN Philadelphia reports.

Klentak said the Phillies are not focused on moving Giles, but they will listen to all offers. If an offer comes that will help speed the Phillies' rebuilding process, then Giles could be on the move. Giles, 25, is attractive to teams because he isn't eligible for arbitration until after the 2017 season and won't be a free agent until after the 2020 season.

Chapman's trade to the Dodgers could be curtailed because of a domestic-violence allegation from late October, Ken Rosenthal of FOX Sports reports.

Chapman's move to Los Angeles was made public Monday but the official transaction will remain in limbo until further announcements are made by the league and the teams involved, which could take multiple weeks.

Rodriguez was outrighted to Triple-A Lehigh Valley on Monday, Stephen Gross of The Morning Call reports.

Rodriguez was on the 40-man roster, but he struggled at the Triple-A level in 2015 to the tune of a 6.32 ERA and more walks than strikeouts in 68.1 innings. He was eventually demoted and ended the year at Double-A, but he should get another chance at the Triple-A level next season.

Chapman has been traded to the Dodgers for two prospects pending a physical, Fox Sports' Ken Rosenthal reports.

Details regarding the prospects will likely surface soon, but the deal would give the Dodgers a premier 1-2 punch in the back of the bullpen assuming that the plan is to pair Chapman with current closer Kenley Jansen. Given the extremely high cost of starting pitching on the free-agent market this winter, the Dodgers appear to be among the teams interested strengthening the bullpen as a way of reducing the impact of back-end starters.

Hoover will be tendered a contract by the Reds on Wednesday.

After a poor 2014 campaign, Hoover bounced back in 2015, going 8-2 with a 2.94 ERA and 1.17 WHIP in 67 appearances. However, his strikeout rate fell dramatically, from 27.3 percent to 19.7 percent, and his walk rate was once again over 4.0 BB/9. He's not the sure-fire closer-in-waiting behind Aroldis Chapman.

Chapman will be tendered a contract by the Reds on Wednesday.

This was a no-brainer, because while Chapman figures to receive more than $10 million in his third year of arbitration, he's worth every penny and more as one of baseball's most dominant closers. The Reds have acknowledged that they are fielding offers for Chapman, and something will likely get done before 2016, though it's not entirely clear who the frontrunners are.

Pomeranz was traded to the Padres on Wednesday, Susan Slusser of the San Francisco Chronicle reports.

Pomeranz underwent arthroscopic shoulder surgery in the offseason, but it appears all has checked out in that regard. The southpaw spent most of the 2015 campaign pitching out of the A's bullpen, but could be a candidate to rejoin the rotation in San Diego.

Almonte was traded to the Rockies on Tuesday in exchange for reliever Tommy Kahnle.

The 21-year-old righty had an impressive 2.42 ERA and 39:12 K:BB in 44.2 innings (mostly as a starter) at High-A last year. That said, as a low-pedigree Rockies pitching prospect who has not yet reached Double-A, he has little value in dynasty leagues.
