Wacha (oblique) was forced to leave Tuesday's rehab start at Double-A Springfield due to fatigue, Jenifer Langosch of MLB.com reports.

Cardinals' Michael Wacha: Leaves Tuesday's rehab start
Wacha (oblique) exited Tuesday's rehab start at Double-A Springfield with an apparent injury, Derek Shore of The Cardinal Nation reports.
Wacha lasted less than three innings before leaving the game, and allowed five earned runs on six hits while throwing 52 pitches. It remains unclear whether the 27-year-old aggravated the oblique strain that landed him on the disabled list in mid-June or if he is dealing with a new injury, but it remains a worrying development regardless.

Chris Paddack RP | TEX
Padres' Chris Paddack: Healthy, shut down for season
Paddack has been shut down for the remainder of the season, Dennis Lin of The Athletic San Diego reports.
The Padres understandably opted to cap Paddack at 90 innings in his first season back from Tommy John surgery. The 22-year-old looked sharp across 17 starts with High-A Lake Elsinore and Double-A San Antonio, posting a combined 2.10 ERA, 120:8 K:BB and 0.82 WHIP. He'll likely report back to San Antonio to start the 2019 season and could push his way to the majors if he keeps on impressing on the farm.

Julio Urias SP | LAD
Dodgers' Julio Urias: Won't join Dodgers on Saturday
Manager Dave Roberts said Urias (shoulder) won't be ready to join the Dodgers when rosters expand Saturday, Ken Gurnick of MLB.com reports.
Urias will need a couple of more outings in the minors before being summoned to the big club. Once he's with the Dodgers, the left-hander is expected to settle into a middle-relief role for the stretch run.

Adam Wainwright SP | STL
Cardinals' Adam Wainwright: Another rehab start scheduled
Wainwright (elbow) will make another rehab start for Triple-A Memphis on Saturday, Rob Rains of StLSportsPage.com reports.
Wainwright worked up to 61 pitches in his most recent rehab start. He'll keep increasing his pitch count, which suggests the Cardinals are considering using him in the rotation in September. If he does regain a starting role right away, he could be lined up to start Sept. 7 in Detroit.

Ross Stripling SP | KC
Dodgers' Ross Stripling: Not yet ready to return
Stripling (back) is not yet ready to return Tuesday, Ken Gurnick of MLB.com reports.
Stripling doesn't feel 100 percent recovered from his lower-back issue. Instead of coming off the disabled list, he'll throw a bullpen session Wednesday. If all goes well, he could return over the weekend.

Cole Irvin RP | LAD
Phillies' Cole Irvin: Named International League Pitcher of the Year
Irvin was named the International League's Pitcher of the Year on Tuesday, Tom Housenick of The Morning Call reports.
Irvin doesn't have all that much prospect pedigree, but he's posted excellent numbers throughout the season for Triple-A Lehigh Valley, recording a 2.67 ERA over 155.1 innings. Scouting reports have him as a back-of-the-rotation, command-driven lefty, as he lacks velocity and has no plus secondary pitches. The 24-year-old achieved great results against Triple-A hitters despite those limitations, however, with a low 5.5 percent walk rate playing a large part in his success. He's not on the Phillies' 40-man roster, so a September callup appears unlikely, but he could be given a look at some point next season.

Ben Lively SP | CLE
Phillies' Ben Lively: Activated from minor-league disabled list
Lively (shoulder) was activated from the minor-league disabled list Tuesday, Tom Housenick of The Morning Call reports.
Lively was expected to be out for the year when he suffered a shoulder injury in mid-July, but it appears that he's beaten that timeline. He remains on the Phillies' 40-man roster, so he has a shot to be a September callup if fully healthy, but little in his track record suggests he'll be a valuable fantasy asset if and when he does return. He recorded a 6.85 ERA in five starts for the Phillies this season and has a 4.81 ERA in 20 career starts overall.

Jose Urena SP | LAA
Marlins' Jose Urena: Activated after suspension ends
Urena was activated from the restricted list Monday after completing his six-game suspension.
Urena was handed the ban after he intentionally hit the Braves' Ronald Acuna on the first pitch of his Aug. 15 start. The right-hander appealed the suspension and returned to the mound four days later to submit a complete-game, two-hitter against the Nationals, only to officially drop the appeal last week. The Marlins will reinsert Urena into the rotation right away, as he's scheduled to start the team's first game of the week Tuesday in Boston.
