Padres' Jason Adam: Carted off Monday
Adam (leg) was carted off the field in the seventh inning of Monday's contest against the Orioles, Bob Nightengale of USA Today reports.
Adam entered the game in the top of the seventh inning with the score tied 3-3, recording one out before surrendering back-to-back singles, the latter of which he suffered a left leg injury during. The right-hander went down in a heap, potentially sustaining the injury on his follow-through. Unfortunately, Adam appears to be facing a substantial absence with a potentially season-ending injury.

Edwin Diaz RP | NYM
Mets' Edwin Diaz: Notches 25th save
Diaz picked up the save in Monday's 10-8 win over the Tigers, throwing 1.1 clean innings. He struck out two.
The save was the 25th of the year for Diaz, who last saw a save opportunity back on Aug. 16 versus the Mariners. The All-Star right-hander had a shaky 4.50 ERA through April 26, but he's essentially been a lights-out closer since the start of May. Over his last 41 innings, Diaz has a 1.10 ERA, 0.83 WHIP and 61:11 K:BB to enhance his ERA to 1.87 for the campaign.

Marlins' Lake Bachar: Ineffective as opener Monday
Bachar (5-2) took the loss in Monday's 2-0 defeat to the Nationals, allowing two runs on two hits and no walks in two innings. He struck out two.
Bachar made his first start of the season as an opener, but he didn't fare well over his two frames of work. It's unclear how the Marlins plan on deploying the right-hander going forward, though he'll likely need a few days off before becoming available from the bullpen again. Through 60.1 frames, Bachar owns a 3.58 ERA, 1.19 WHIP and 67:27 K:BB.

Jose Ferrer RP | WAS
Nationals' Jose Ferrer: Tallies fifth save Monday
Ferrer earned the save in Monday's 2-0 win over the Marlins, allowing no hits and one walk in a scoreless ninth inning. He struck out one.
Ferrer produced his fifth save of the season, and each of which of have come following the trade of Kyle Finnegan. Ferrer had a 5.28 ERA through June 29, but he's settled in as Washington's closer and established a fairly strong grip on the role. Over his last 21.2 innings, the southpaw boasts a 1.66 ERA, 1.06 WHIP and 21:3 K:BB since the start of July to bring his ERA to 4.09 for the year.

Shelby Miller RP | MIL
Brewers' Shelby Miller: Dealing with elbow issue
Miller exited Monday's 10-8 loss to the Phillies early with a right elbow injury, Adam McCalvy of MLB.com reports.
Miller felt something "pop" in his elbow, leading to an early departure, which certainly doesn't sound good. The right-hander may be set for an extended absence, but more information on his status will likely come in the near future.

Diamondbacks' Cristian Mena: Yet to throw off mound
Mena (shoulder) has continued his throwing program, but he has yet to throw off a mound, Alex Weiner of ArizonaSports.com reports.
Mena has been out since early June after suffering a right shoulder strain, and he doesn't appear to be particularly close to returning to action. With less than a month left in the 2025 campaign, the right-hander may be running short on time. Mena will likely have to participate in multiple bullpen sessions and an eventual minor-league rehab assignment before he'd be able to return.

Andrew Hoffmann RP | ARI
Diamondbacks' Andrew Hoffmann: Out of action at Triple-A
Hoffmann hasn't pitched since being optioned to Triple-A Reno on Aug. 20 while dealing with fatigue, Alex Weiner of ArizonaSports.com reports.
Reno hasn't yet placed Hoffmann on its injured list, so he could be back in action within a few days. Hoffmann has posted a 6.55 ERA and 2.00 WHIP over 11 innings in 10 appearances at the MLB level with the Royals and Diamondbacks this season.

Anthony DeSclafani RP | ARI
Diamondbacks' Anthony DeSclafani: Set to face hitters
DeSclafani (thumb) is expected to face live hitters Tuesday, Alex Weiner of ArizonaSports.com reports.
DeSclafani has been sidelined since Aug. 13 while dealing with right thumb inflammation, but he will now take a significant step in his recovery. If Tuesday's session goes well, the Diamondbacks will likely start to put together the framework for a minor-league rehab assignment for DeSclafani in the coming weeks.

Shelby Miller RP | MIL
Brewers' Shelby Miller: Leaves game with trainer
Miller exited Wednesday's game against the Phillies in the top of eighth inning with an apparent injury, Jack Stern of BrewerFanatic.com reports.
Miller threw eight pitches in the top of the eighth inning, allowing one run on two hits, before the Brewers' trainer came out to the mound to examine the right-hander. Miller was removed from the game and replaced Rob Zastryzny. Milwaukee has an off day Tuesday before resuming its series with Philadelphia on Wednesday.

Riley O'Brien RP | STL
Cardinals' Riley O'Brien: Plays catch, could avoid IL
O'Brien (shoulder) was cleared to play catch Monday and could avoid the 15-day injured list, Daniel Guerrero of the St. Louis Post-Dispatch reports.
O'Brien underwent an MRI on Monday due to lingering right shoulder discomfort, but the Cardinals were encouraged by the imaging results and the reliever was allowed to resume throwing. While a stint on the IL has not been ruled out, O'Brien will be given a couple days to continue to recover to see if a long-term absence can be avoided. The 30-year-old has emerged as a late-inning bullpen arm for St. Louis this season, collecting a 1.69 ERA and 35:17 K:BB over 37.1 innings while notching two saves.
