  • Blue Jays' Alek Manoah: Rehab stint extended

Toronto has extended Manoah's (elbow) minor-league rehab assignment by an extra 10 days, Ben Nicholson-Smith of Sportsnet.ca reports. Manoah's initial 30-day rehab stint came to a close, but the right-handed hurler isn't yet ready to be activated. The Blue Jays have thus elected to use one of a potential three 10-day extensions available for pitchers coming back from Tommy John surgery per the collective bargaining agreement. Manoah has struggled during his rehab assignment to this point, posting a 7.71 ERA, 2.06 WHIP and 10:10 K:BB over 11.2 innings spanning five starts, though his most recent performance was his best -- he allowed one run over 4.2 frames with four strikeouts Wednesday with Triple-A Buffalo. Nicholson-Smith indicates that Manoah will throw a side session this weekend and is slated for another start with Buffalo early next week.

  • Blue Jays' Alek Manoah: Lacks control in latest rehab start

Manoah (elbow) struck out one batter over 1.2 innings and 49 pitches in his rehab start Wednesday for Triple-A Buffalo while allowing three earned runs on two hits, three walks and a hit batsman. Making his fourth rehab start with his fourth different affiliate, Manoah displayed wayward control, which has been a recurring theme since he kicked off his minor-league assignment July 14. He now holds an 11.57 ERA, 2.71 WHIP and 6:8 K:BB across seven innings through four starts, showing little resemblance to the pitcher who earned an All-Star nod in 2022. Manoah's 30-day rehab window has come to a close, though as Arden Zwelling of Sportsnet.ca notes, the Blue Jays will have the ability to extend his assignment in 10-day increments until his rehab reaches 60 days. The 27-year-old still has two minor-league options remaining, and given the poor form he's displayed thus far on his rehab assignment, he'll likely stay with Buffalo once Toronto formally activates him from the 60-day injured list.

  • Blue Jays' Alek Manoah: Makes start at Double-A

Manoah allowed two earned runs on five hits and three walks while striking out two across three innings in a rehab outing with Double-A New Hampshire on Thursday, Mitch Bannon of The Athletic reports. Manoah has been pitching in a combination of rehab outings and simulated games across the last three weeks. While the numbers aren't overwhelmingly positive with a 10.13 ERA and 5:5 K:BB over 5.1 rehab innings, Thursday's appearance marked Manoah's longest both in terms of outs recorded and pitches thrown (64). It's unclear how many more rehab starts he'll need, but Manoah could be ready for his 2025 MLB debut by the end of August.

  • Blue Jays' Alek Manoah: Builds up to 45 pitches

Manoah (elbow) completed a 45-pitch, three-inning simulated game Saturday and is scheduled to make his third rehab start later this week with Single-A Dunedin, Arden Zwelling of Sportsnet.ca reports. After Manoah made rehab starts with the Blue Jays' rookie-level Florida Complex League affiliate July 14 and with Dunedin on July 20, Toronto opted to have the right-hander pitch in a controlled setting over the weekend. He was able to build up his pitch count during the sim game and flashed a fastball that topped out at around 95 miles per hour, so Manoah seems to be in a good spot as he works his way back from the internal brace procedure he underwent last June. Manoah will throw a bullpen session Tuesday before making his next start with Dunedin, and if all goes well in that outing, he'll shift his rehab assignment to a higher-level affiliate next week. Manoah appears to be tracking toward a return from the 60-day injured list around mid-to-late August, but the Blue Jays may not have a rotation spot available for him once he's activated.

  • Blue Jays' Alek Manoah: Building up for August

Manoah (elbow) will pitch a simulated game Saturday, Keegan Matheson of MLB.com reports. Single-A Dunedin is on the road Saturday, so the Blue Jays are opting for a simulated game in which they can control Manoah's pitch count instead. The right-hander should be ready to return to full capacity by the end of August or early September.

  • Blue Jays' Alek Manoah: Sharper in second rehab start

Manoah (elbow) gave up one run on three hits over two innings for Single-A Dunedin on Sunday. He struck out two without walking a batter. The right-hander tossed 28 pitches (16 strikes) in his second rehab outing as he enters the final stretch of his recovery from an internal brace procedure performed last June. Per Mitch Bannon of The Athletic, Manoah will need at least three more rehab starts before the Blue Jays will consider activating him from the 60-day IL, but at the moment there isn't a spot for him in the big-league rotation, and the team may well have added further depth at the trade deadline before he's ready to return.

  • Blue Jays' Alek Manoah: Second rehab start set

Manoah (elbow) will make a rehab start Sunday with Single-A Dunedin, Shi Davidi of Sportsnet.ca reports. The right-hander surrendered four runs (three earned) while recording just one out during his first rehab outing Monday, and he'll attempt to ramp up his workload this weekend. Manoah will likely need at least a couple additional starts in the minors after Sunday before being considered for activation.

  • Blue Jays' Alek Manoah: Struggles in first rehab start

Manoah (elbow) allowed four runs (three earned) on one hit and two walks while recording just one out in his first rehab start in the rookie-level Florida Complex League on Monday. Manoah also hit a batter and was charged with a wild pitch while spotting just 10 of 22 pitches for strikes in the outing. The performance was a discouraging one as the big righty works his way back from internal brace surgery, but Manoah and the Blue Jays aren't necessarily worried about results at this early stage of his rehab assignment. He'll need to complete multiple rehab starts before warranting consideration for a spot in Toronto's rotation by early August.

  • Blue Jays' Alek Manoah: Set for live BP

Manoah (elbow) is expected to throw a live BP session Tuesday, Shi Davidi of Sportsnet.ca reports. Manoah will throw a two-up live batting practice session, meaning that he'll essentially toss two innings of simulated action. The right-hander has been out for the entirety of the 2025 campaign to this point while recovering from a right elbow procedure he underwent in June 2024. Manoah is expected to require a multi-start minor-league rehab assignment, which could begin as soon as next week.

  • Blue Jays' Alek Manoah: Continues ramping up

Manoah (elbow) is set to throw two innings in a bullpen session Tuesday at the Blue Jays' spring training complex, Arden Zwelling of Sportsnet.ca reports. The right-hander threw 25 pitches in a one-inning session June 24, but next Tuesday's session will be the first time he's been asked to handle multiple up-downs. Manoah could begin a rehab assignment afterward as he enters the final stage of his recovery from last June's internal brace procedure on his right elbow, which would likely have him on schedule to be a rotation option for Toronto some time in August.

  • Blue Jays' Alek Manoah: First mound session since surgery

Manoah (elbow) will throw a bullpen session Friday, Keegan Matheson of MLB.com reports. It will be the first time the right-hander has thrown off a mound since he underwent a hybrid internal brace procedure on his elbow last June. The plan calls for Manoah to face hitters by June and begin a rehab assignment in July, setting him up to help out the Blue Jays by August if all goes well.

  • Blue Jays' Alek Manoah: Close to throwing off mound

Manoah (elbow) said Monday that he's scheduled to begin throwing off a mound in a couple weeks, Hazel Mae of Sportsnet reports. Manoah underwent a hybrid internal brace procedure last June to repair a torn UCL in his right elbow. The right-hander is hopeful to face hitters by June, begin a rehab assignment in July and be available as an option for the Blue Jays in August. In order to meet his goals, Manoah will have to avoid any bumps in the road in his recovery, which isn't always a guarantee coming back from UCL surgery.

  • Blue Jays' Alek Manoah: Reaches deal with Toronto

Manoah (elbow) agreed to a one-year, $2.2 million contract with the Blue Jays on Thursday to avoid arbitration, Shi Davidi of Sportsnet.ca reports. Following a delayed start to the season, Manoah was able to turn in a 3.70 ERA and 1.03 WHIP over 24.1 innings before a sprained UCL ended his campaign early in late May. The 27-year-old underwent a hybrid internal brace procedure to address the issue in June and is targeting August for his return to a big-league mound in 2025.

  • Blue Jays' Alek Manoah: Aiming to return in August

Manoah (elbow) hopes to be ready to pitch for the Blue Jays in August, Mike Wilner of the Toronto Star reports. Manoah underwent a hybrid internal brace procedure to repair the UCL in his right elbow last June. The plan calls for Manoah to throw bullpen sessions in March, face hitters in June and go out on a rehab assignment in July before rejoining the Blue Jays in August. If the righty is able to hit those markers, he could spend most of the final two months of the 2025 campaign in Toronto's rotation.

  • Blue Jays' Alek Manoah: Sheds protective brace

Manoah revealed Monday in a social media post that he had the brace removed from his surgically repaired right elbow. Manoah underwent an internal brace procedure six weeks ago to address ligament damage to his elbow. While Manoah will be out for the season and for the start of the 2024 campaign, he could face a speedier recovery from his procedure than if he had opted for Tommy John surgery, which often entails a 12-to-14-month rehab timeline for pitchers. The Blue Jays likely won't have a better feel for when Manoah might be ready to make his 2025 debut until seeing where he stands in his rehab program next spring.

  • Blue Jays' Alek Manoah: Set for internal brace surgery

Manoah won't require a traditional Tommy John surgery and is instead slated to undergo a hybrid internal brace procedure Monday to repair the sprained UCL in his right elbow, Shi Davidi of Sportsnet.ca reports. Blue Jays manager John Schneider had previously disclosed Friday that Manoah would require a season-ending elbow surgery, but Davidi's report provides more specificity regarding the nature of the procedure. Renowned arm specialist Dr. Keith Meister is set to perform the procedure, which involves replacing the UCL and anchoring the new UCL to an internal brace suture. Though the internal brace procedure typically shortens the length of a pitcher's recovery compared to the 12-to-18-month timeline for Tommy John surgery, Davidi notes that Manoah is still expected to be on the shelf "well into next year." The significant arm injury further muddles Manoah's long-term outlook with the Blue Jays, which had already been somewhat murky following a disastrous 2023 campaign in which he posted a 5.87 ERA and 14.2 percent walk rate over 87.1 innings.

  • Blue Jays' Alek Manoah: Set to undergo UCL surgery

Manager John Schneider said Friday that Manoah will undergo UCL surgery June 17, Shi Davidi of Sportsnet.ca reports. Manoah left in the second inning of his last start May 29 against the White Sox, and he was placed on the injured list two days later with what was diagnosed as a right UCL sprain. Manoah began the season on the IL with right shoulder inflammation, and since returning May 5, he's posted a 1-2 record with a 3.70 ERA, 1.03 WHIP and 26:8 K:BB over 24.1 innings. It's not clear whether Manoah will need Tommy John surgery or a brace repair, but either procedure would end the 26-year-old right-hander's 2024 season.

  • Blue Jays' Alek Manoah: Diagnosed with sprained UCL

The Blue Jays placed Manoah on the 15-day injured list Friday, retroactive to May 30, with a right elbow sprain, Shi Davidi of Sportsnet.ca reports. Manoah has damage to his UCL and will receive a second opinion from Dr. Keith Meister next Thursday, per Ben Nicholson-Smith of Sportsnet.ca. Season-ending surgery looks like a distinct possibility, but that decision should be made after next week's exam. Manoah has posted a 3.70 ERA, 1.03 WHIP and 26:8 K:BB over 24.1 innings covering five starts with the Blue Jays this season.

  • Blue Jays' Alek Manoah: Expected to miss significant time

Manoah will receive a second opinion on his injured right elbow, but the expectation is that he's going to be sidelined for an extended period of time, Scott Mitchell of TSN.ca reports. Manoah was forced from his last outing Wednesday against the White Sox in the second inning due to discomfort in his pitching elbow. He underwent an MRI on Thursday, and it would appear the results of that exam were not favorable. Manoah has not yet been placed on the injured list, but that looks to be inevitable. Bowden Francis (forearm) is slated to return soon and is an option to take Manoah's rotation spot.

  • Blue Jays' Alek Manoah: Falls to Detroit

Manoah (1-2) took the loss Friday against the Tigers, allowing six runs (four earned) on five hits and two walks over 4.2 innings. He struck out four. It was a tough outing for Manoah, who hadn't allowed an earned run over 14 innings in his previous two starts. Still, the 26-year-old right-hander has gotten off to a solid start in 2024, pitching to a 3.97 ERA with a 1.06 WHIP and 23:8 K:BB through 22.2 innings after struggling to a 5.87 ERA in 19 starts last year. Manoah will look to get back on track in his next outing, currently lined up for next week on the road versus the White Sox.
