Guardians' Trevor Stephan: Removed from 40-man roster
The Guardians outrighted Stephan on Friday, Tim Stebbins of MLB.com reports.
Stephan has been playing for Triple-A Columbus since being activated off the 60-day injured list July 30, and he'll remain at Columbus or elect free agency after being outrighted. The 29-year-old has allowed 11 runs and three homers in seven innings with Columbus in 2025.

Guardians' Trevor Stephan: Activated and optioned
The Guardians activated Stephan (elbow) from the 60-day injured list and optioned him to Triple-A Columbus on Wednesday.
Stephan was one of the Guardians' most-trusted relievers in 2022 and 2023, but he's struggled during his rehab assignment while coming back from his March 2024 UCL surgery, posting an 11.81 ERA over 10.2 innings across stops with three affiliates. He can still make an impact with the big club later this season, but the Guardians will need Stephan to hone his command at Triple-A before giving him another look in the majors.

Guardians' Trevor Stephan: Restarts rehab assignment
Stephan (elbow) resumed his rehab assignment at Triple-A Columbus over the weekend, issuing one walk in a scoreless inning Friday.
Stephan is in the midst of his second rehab assignment after the Guardians opted to extend his stay in the minors when his initial 30-day window expired shortly before the All-Star break. During the initial assignment, Stephan made eight appearances across stops with three affiliates and logged a 5.40 ERA and 1.44 WHIP in 8.1 innings. Stephan has made two appearances for Columbus during his second assignment, with Friday's outing marking his first since the All-Star break. The right-hander will likely need to sharpen his command and pitch on consecutive days at Triple-A before the Guardians bring him back from the 60-day injured list.

Guardians' Trevor Stephan: Shifts to Double-A
Stephan (elbow) has had his rehab assignment moved to Double-A Akron, MLB.com reports.
Stephan made two appearances in the Arizona Complex League and two for Triple-A Columbus before the organization shifted him to Akron. He threw two scoreless innings for the RubberDucks on Tuesday, allowing one hit while striking out two. Guardians' Trevor Stephan: Close to game action in minors
Guardians manager Stephen Vogt said Sunday that Stephan (elbow) is "progressing well" and is expected to return to game activity at the organization's spring training complex in Arizona at some point in May, Michael Beaven of the Akron Beacon Journal reports.
Stephan missed the entire 2024 season while recovering from UCL reconstruction surgery, which he underwent last March. Now more than a year removed from the procedure, Stephan has reached the point in his recovery program that he's facing hitters, and he'll likely soon test himself in a simulated-game setting in Arizona before reporting to an affiliate for a rehab assignment. The right-hander was a key piece of the Cleveland bullpen in his last healthy season in 2023, turning in a 4.06 ERA, 1.30 WHIP and 75:26 K:BB while collecting seven wins, 30 holds and two saves over 71 appearances.

Stephan underwent season-ending surgery Wednesday to repair the torn UCL in his right elbow, Mandy Bell of MLB.com reports. The Guardians placed him on the 60-day injured list Thursday.
Though it's not clear if Stephan underwent Tommy John surgery or an internal bracing procedure on his elbow, he'll still be ticketed for a 12-to-16-month rehab and recovery. His move to the 60-day IL freed up a spot on the 40-man roster for right-hander Tyler Beede, whose contract was selected from Triple-A Columbus.

The specific procedure and a timetable aren't yet known, but Bell relays that Stephan's UCL was "not providing adequate stability," necessitating the need for the operation. More information on a recovery timeline for Stephan should be known once he goes under the knife, but unless he gets an arthroscopic procedure, the veteran reliever could miss the entire 2024 season. Stephan will be shut down for three weeks with a deep bone bruise in his right elbow, Zack Meisel of The Athletic reports.
Stephan's elbow is structurally sound, but the Guardians will shelve the reliever for a while to let him recover from the bone bruise. Considering that Opening Day is just over four weeks away, Stephan undoubtedly will require a stay on the injured list. The 28-year-old has been a key setup man for Cleveland, posting a 3.73 ERA and 27.5 percent strikeout rate over the last three seasons.

Stephan (7-7) allowed two runs on three hits and struck out one over one inning to earn the win despite taking a blown save Thursday versus the Orioles.
Stephan allowed the Orioles to pull even in the top of the eighth inning, but the Guardians retook the lead in the bottom of the frame. It's been a rough September for the setup man -- he's allowed 12 runs (11 earned) over 7.1 innings across eight appearances. He has a 3.97 ERA, 1.29 WHIP and 71:25 K:BB with two saves, 29 holds and eight blown saves through 65.2 innings overall.

Stephan (6-7) allowed four runs (three earned) on three walks and a hit-by-pitch over two-thirds of an inning to take the loss Monday versus the Royals.
Stephan couldn't find the strike zone, with just 12 of his 31 pitches landing for strikes in the wild outing. His poor showing allowed the Royals to rally in the eighth inning, though Enyel De Los Santos was tagged with the blown save after being unable to get the situation under control. Stephan allowed a go-ahead RBI single to Taylor Walls in the eighth inning. It was just the third run he's allowed over 15 innings since the start of August. The right-hander is now at a 2.83 ERA, 1.21 WHIP and 67:20 K:BB through 60.1 innings while adding two saves and 25 holds. While he'll still likely see plenty of setup work, the waiver additions of Matt Moore and Reynaldo Lopez could impact Stephan's usage over the last month of the season.
