Reds' Carson Spiers: Needs Tommy John surgery
Reds manager Terry Francona said Saturday that Spiers will require season-ending Tommy John surgery on his right elbow, Pat Brennan of The Cincinnati Enquirer reports.
Spiers had been in the midst of a rehab assignment at Triple-A Louisville while working his way back from a shoulder impingement, but he was pulled off the assignment about a week ago after experiencing soreness in his right biceps. He underwent a couple of MRIs since then, and a second opinion confirmed that Tommy John surgery was necessary. No date has been set for Spiers' procedure, but the surgery will be performed by renowned arm specialist Dr. Keith Meister. In addition to missing the remainder of the season, Spiers will be at risk of missing the entire 2026 campaign as well.

Reds' Carson Spiers: MRI reveals ligament issues
Spiers (shoulder) underwent an MRI on Monday that revealed ligament issues, MLB.com reports.
Reds manager Terry Francona confirmed the results of the MRI and said Spiers will seek a second opinion. "It's not cut and dry, but there is some concern," Francona said. Spiers first hurt the shoulder in April and was transferred to 60-day injured list in late May.

Reds' Carson Spiers: Pulled from rehab stint
The Reds have returned Spiers (shoulder) from his minor-league rehab assignment due to right bicep soreness, Charlie Goldsmith of Fox 19 Now Cincinnati reports.
Spiers landed on the injured list due to a right shoulder impingement April 20 and was transferred to the 60-day IL in late May. He appeared to be nearing activation, as the right-hander began a rehab assignment July 3 and worked up to 75 pitches over 3.1 frames in his most recent outing Saturday, but he's experienced a setback due to an issue with his right bicep. It's unclear how much more time Spiers will need to miss as a result.

Reds' Carson Spiers: Goes 3.2 scoreless in rehab start
Spiers (shoulder) struck out five and gave up one hit and four walks over 3.2 scoreless innings in his rehab start for Triple-A Louisville on Sunday.
Though Spiers kept Columbus off the board during his start, he spotted just 39 of his 72 pitches for strikes in the outing, which was the third of his rehab assignment. On the shelf since April 20 due to a right shoulder impingement, Spiers appears to be close to fully stretched out at this point, but the Reds don't seem to have an opening for him in the big-league rotation. Spiers could be activated from the 60-day injured list coming out of the All-Star break to serve as a long-relief option for the big club, or the Reds could option him to Triple-A if they want to keep him in a starting role.

Reds' Carson Spiers: Rehab moving to Triple-A
Spiers (shoulder) is expected to have his rehab assignment shifted from the rookie-level Arizona Complex League to Triple-A Louisville on Tuesday, MLB.com reports.
Spiers made his lone appearance in the ACL on Wednesday, allowing two hits while striking out four and hitting a batter over 2.1 scoreless innings. He threw 49 pitches in that outing and will look to build on that workload Tuesday, when he's expected to piggyback fellow rehabbing right-hander Hunter Greene (groin). Spiers look on track to rejoin the Reds after the All-Star break and will likely fill a multi-inning role out of the bullpen after he started in two of his three appearances for Cincinnati before landing on the shelf April 20 due to a right shoulder impingement.

Reds' Carson Spiers: Rehab assignment scheduled
Spiers (shoulder) will begin a rehab assignment July 3 with the Reds' affiliate in the Arizona Complex League, MLB.com reports.
Spiers will be limited to three innings in his first outing for the ACL Reds. The right-hander has been on the injured list -- first the 15-day before shifting to the 60-day IL -- since April 20 with what was described as a shoulder impingement. Given the need for Spiers to build up innings and pitch counts, he's looking at multiple rehab starts before activation.

Reds' Carson Spiers: Throws bullpen session
Spiers (shoulder) threw a second bullpen session Tuesday, MLB.com reports.
Spiers has been sidelined with a right shoulder impingement since mid-April and began a throwing program three weeks ago. The next step should be a rehab assignment, although the Reds haven't indicated when that may begin.

Reds' Carson Spiers: Shifted to 60-day injured list
The Reds transferred Spiers (shoulder) to the 60-day injured list Friday.
The move clears a spot on the 40-man roster for the addition of Garrett Hampson. Spiers recently began a throwing program as he comes back from a right shoulder impingement, but he now will not be eligible for activation until late June.

Reds' Carson Spiers: Begins throwing Saturday
Reds manager Terry Francona said that Spiers (shoulder) began a throwing progression Saturday at the team's spring training facility in Arizona, Charlie Goldsmith of the Dayton Daily News reports.
Spiers will likely remain in Arizona for at least the next couple of weeks to continue ramping up before the Reds map out a minor-league rehab assignment for him. The 27-year-old righty had started in two of his three appearances for Cincinnati before landing on the shelf April 20 with a right shoulder impingement, but he might have to settle for a long-relief role once he's activated.

Reds' Carson Spiers: No longer starting Sunday
Spiers pitched out of the bullpen in Saturday's loss to the Orioles, allowing four runs on five hits and four walks while striking out three over four innings.
Spiers was originally promoted Friday for the purpose of starting Sunday in place of Nick Lodolo (paternity), but manager Terry Francona changed those plans on the fly, calling upon Spiers for mop-up duty after Hunter Greene allowed five runs over three innings Saturday. "(Spiers) wore it like a trooper," Francona told Charlie Goldsmith of the Dayton Daily News after Saturday's game while declining to name a starter for the series finale. Goldsmith suggests the Reds will need to make at least one roster move to bolster the pitching staff prior to Sunday's game, and Spiers is almost certainly headed back to Triple-A Louisville for the time being.

Reds' Carson Spiers: Out with shoulder impingement
The Reds placed Spiers on the 15-day injured list Sunday due to a right shoulder impingement, Charlie Goldsmith of the Dayton Daily News reports.
The 27-year-old was called up Friday and was scheduled to start Sunday in place of Nick Lodolo (paternity), but Hunter Greene's short outing Saturday resulted in Spiers covering four frames out of the bullpen. The right-hander apparently tweaked his shoulder while giving up four runs, so he'll be sidelined for at least the next couple weeks. Randy Wynne was selected to the MLB roster Sunday and could provide some length behind opener Brent Suter against the Orioles.

Reds' Carson Spiers: Sent down to Triple-A
The Reds optioned Spiers to Triple-A Louisville on Tuesday.
Spiers had a solid start to begin his season, allowing one earned run over six innings of work April 1. However, he allowed four earned across 3.1 innings in his following start Sunday. The 27-year-old will return to Triple-A Louisville, where he posted a 2.51 ERA across 46.2 innings in 2024. With Spiers' demotion and Matt McLain (hamstring) landing on the injured list, Will Benson and Noelvi Marte were both recalled from Triple-A Louisville on Tuesday.

Reds' Carson Spiers: Ineffective versus Brewers
Spiers (0-2) pitched 3.1 innings, allowing four runs on five hits and a walk while striking out three batters on his way to a loss in Sunday's 8-2 defeat to the Brewers.
Spiers was able to pitch two scoreless frames before running into trouble in the third inning. He surrendered three hits and two runs in the frame before the wheels fell completely off in the fourth. After giving up back-to-back singles to open the inning, Spiers was pulled from the game, and both of the runners later came around to score. The right-hander has now given up five runs over 9.1 innings in two starts this season, and with Andrew Abbott (shoulder) eligible to return to the rotation Saturday, it's unclear if Spiers will make another start or transition into being a bullpen arm.

Reds' Carson Spiers: Throws final Cactus League outing
Spiers allowed one run on four hits while striking out one over four relief innings in Saturday's spring start against San Diego.
He entered after Andrew Abbott in what was Spiers' final Cactus League outing before he opens the regular season with a spot in the rotation. Spiers closed out spring training with a 2.57 ERA through 14 innings over four appearances (one start).

Reds' Carson Spiers: Opening season in rotation
Spiers will open the season as the Reds' fifth starter, Gordon Wittenmyer of The Cincinnati Enquirer reports.
It could be temporary until Andrew Abbott (shoulder) is ready, but Spiers will have at least a couple starts to convince the Reds he should remain in the rotation. The right-hander has had a strong showing this spring, collecting a 2.70 ERA and 10:4 K:BB across 10 frames.

Reds' Carson Spiers: Logs five innings
Spiers allowed one run on one hit and two walks while striking out two over five relief innings in Monday's Cactus League game against Cleveland.
Spiers is on the outside looking in at the Reds' projected rotation, but he remains on call if needed. The right-hander pitched in relief of starter Andrew Abbott, who made his spring debut Monday and is not certain to be ready to go when the regular season kicks off. "You know what? This kid has had a heck of a camp. He's filling up the strike zone and changes speed," Cincinnati manager Terry Francona said of Spiers' spring to Mark Sheldon of MLB.com. "I don't know him as well as the rest of these guys do, but he's handled himself really well." Spiers has a 2.70 ERA and allowed five hits and four walks while striking out 10 over 10 Cactus League innings.
