  • Guardians' Carlos Hernandez: Accepts outright assignment

Hernandez cleared waivers and accepted his outright assignment to Triple-A Columbus on Friday, Tim Stebbins of MLB.com reports. The Guardians designated Hernandez for assignment Wednesday, and he'll stick around in the team's farm system after clearing waivers. The 28-year-old has been DFA'd three times this season and has a 6.23 ERA in 43.1 big-league innings combined during his stints with the Guardians, Tigers and Phillies in 2025.

  • Guardians' Carlos Hernandez: DFA'd by Guardians

The Guardians designated Hernandez for assignment Wednesday. The transaction frees up spots on the active and 40-man rosters for Parker Messick, who's been summoned from Triple-A Columbus to start Wednesday's game in Arizona. Hernandez has allowed three runs with a 3:3 K:BB over seven innings since arriving in Cleveland via waiver claim and will now go through the waivers process again.

  • Guardians' Carlos Hernandez: Claimed by Guardians

The Guardians claimed Hernandez off waivers from the Tigers on Thursday, Tim Stebbins of MLB.com reports. Hernandez was pushed off the Tigers' 40-man roster last Thursday after surrendering 13 runs (12 earned) in just 10.2 innings. He'll now reclaim a 40-man roster spot upon moving to the Guardians organization, though he'll likely have to redeem himself in Triple-A before he gets another shot in the bigs.

  • Tigers' Carlos Hernandez: Designated for assignment

The Tigers designated Hernandez for assignment Thursday. Hernandez was a waiver claim of the Tigers in June and is now headed back to waivers after he coughed up 13 runs (12 earned) over 10.2 frames for Detroit. The righty has been unable to translate his plus velocity into consistent results at the major-league level, with his career 11.1 percent walk rate being a big reason for that.

  • Tigers' Carlos Hernandez: Claimed by Detroit

The Tigers claimed Hernandez off waivers from the Phillies on Monday. Hernandez had been designated for assignment after logging a 5.26 ERA, 1.75 WHIP and 23:13 K:BB in 25.2 innings for the Phillies. The hard-throwing right-hander does not have minor-league options remaining, so he will join the Tigers' active roster. Detroit will announce a corresponding transaction prior to Tuesday's game against the Pirates.

  • Phillies' Carlos Hernandez: Dropped from 40-man roster

The Phillies designated Hernandez for assignment Wednesday. Hernandez doesn't have any minor-league options remaining, so the Phillies had to remove him from their 40-man roster to clear a spot for right-hander Michael Mercado, who was recalled from Triple-A Lehigh Valley in a corresponding move. After being claimed off waivers from the Royals in the spring, Hernandez had operated as a low-leverage reliever for Philadelphia this season, logging a 5.26 ERA, 1.75 WHIP and 23:13 K:BB in 25.2 innings. Any team willing to trade for or put in a waiver claim for the right-hander will have to assume the money remaining on his $1.16 million contract for 2025.

  • Phillies' Carlos Hernandez: Claimed by Philly

The Phillies claimed Hernandez off waivers from the Royals on Sunday. The right-hander was put on outright waivers after missing out on Kansas City's Opening Day bullpen Saturday, and it didn't take long for him to be scooped up by another team. Hernandez had a 3.30 ERA, 1.33 WHIP and 27:16 K:BB over 30 innings during the regular season last year, but he struggled in spring training with eight runs allowed and an 8:5 K:BB over 10.1 innings.

  • Royals' Carlos Hernandez: Doesn't make KC bullpen

Hernandez will not make the Kansas City bullpen and is set to be put on outright waivers, Anne Rogers of MLB.com reports. He's out of minor-league options and is likely to be claimed by another team. While he had a 6.97 ERA this spring with a 8:5 K:BB ratio in 10.1 innings, another team may take a chance on his near 100-mph fastball.

  • Royals' Carlos Hernandez: Settles with Kansas City

The Royals and Hernandez avoided arbitration by agreeing to a one-year, $1.16 million contract Thursday, Anne Rogers of MLB.com reports. It was the second year of arbitration eligibility for Hernandez, who posted a 3.30 ERA and 27:16 K:BB across 30 regular-season innings during 2024. The righty has teased with his big fastball but has shown no indication to this point that he can be consistently relied upon.

  • Royals' Carlos Hernandez: Moves up from Triple-A

The Royals recalled Hernandez from Triple-A Omaha on Monday. He'll absorb the roster spot vacated by Will Smith (back), who was placed on the injured list. Hernandez has permitted eight runs (six earned) with a 14:9 K:BB over 15.2 frames this season during his time with the big club.

  • Royals' Carlos Hernandez: Optioned to Triple-A

The Royals optioned Hernandez to Triple-A Omaha on Monday, Anne Rogers of MLB.com reports. Hernandez allowed just one earned run with a 6:1 K:BB over four innings during his most recent stint in the majors but nonetheless is being sent out. John Schreiber is back from the injured list to absorb his roster spot.

  • Royals' Carlos Hernandez: Returns to majors

The Royals recalled Hernandez from Triple-A Omaha on Saturday. With Hunter Harvey on the injured list due to a back injury, Hernandez will return to Kansas City to fill the open spot in the Royals' bullpen. The 27-year-old righty owns a 3.86 ERA and 1.54 WHIP through 11.2 innings in the majors this year, though he has surrendered seven runs in just 7.2 frames since being optioned to Omaha in early July.

  • Royals' Carlos Hernandez: Handed loss Tuesday

Hernandez (0-1) took the loss versus the Marlins on Tuesday, allowing two runs (one earned) on three hits while striking out one over two-thirds of an inning. Hernandez hadn't given up a run over his first 3.2 innings back in the majors since his recall June 18. That changed in the seventh inning Tuesday, as the Marlins got RBI singles from Nick Gordon and Jazz Chisholm to flip the script on the Royals. Hernandez has allowed three runs (two earned) with a 3:4 K:BB over seven innings across eight appearances this year. While initially thought to be an option for high-leverage work, Hernandez suffered a shoulder impingement in spring training and has yet to record a save or hold in the majors this year.

  • Royals' Carlos Hernandez: Back in majors

The Royals recalled Hernandez from Triple-A Omaha on Tuesday. The right-hander was optioned to Triple-A in late May but will now rejoin the MLB club. Hernandez has made just four big-league appearances this season and has allowed one earned run with one strikeout and three walks over 2.2 innings.

  • Royals' Carlos Hernandez: Clean inning in season debut

Hernandez pitched a perfect inning in Friday's 8-1 win over the Rays. Hernandez got three straight flyouts from Jonathan Aranda, Randy Arozarena and Jose Caballero to finish off the lopsided game. Hernandez missed nearly two months to begin 2024 as well as all of spring training due to a pinched nerve in his right shoulder. He pitched to an 8.22 ERA, 1.94 WHIP and 12:6 K:BB over 7.2 innings during his rehab assignment with Triple-A Omaha, though he kept runs off the board in five of his last six outings before he was activated. The Royals' bullpen has been OK this year, with James McArthur and John Schreiber leading the way for high-leverage opportunities, so Hernandez is likely to be eased in for his first few appearances.

  • Royals' Carlos Hernandez: Back from injured list

The Royals reinstated Hernandez (shoulder) from the 15-day injured list Friday. Hernandez was shut down at the start of spring training due to a pinched nerve in his right shoulder, but he's ready to join the Royals after an eight-game rehab assignment with Triple-A Omaha. The right-hander recorded four saves and 17 holds last season but had a 5.27 ERA and 1.33 WHIP across 70 innings.

  • Royals' Carlos Hernandez: Sent out on rehab assignment

Hernandez (shoulder) began his rehab assignment with Triple-A Omaha on Friday. Omaha's game Friday was rained out, but Hernandez should be expected to make a few rehab outings with the club over the next few weeks as he looks to get up to speed. The right-hander missed all of spring training due to a right shoulder impingement.

  • Royals' Carlos Hernandez: Nearing rehab assignment

Hernandez (shoulder) is expected to begin a rehab assignment later this week, Anne Rogers of MLB.com reports. Hernandez has been throwing since mid-March and is now nearly ready for game action. Given how long he's been out, he'll likely need a handful of rehab outings to get up to speed. Hernandez figures to enter the high-leverage mix fairly quickly once he's healthy, as the Royals' top late-inning options have ranged from fine (James McArthur, John Schreiber) to awful (Will Smith) early in the season.

  • Royals' Carlos Hernandez: Lands on 15-day injured list

The Royals placed Hernandez on the 15-day injured list Thursday with a right shoulder impingement. Hernandez missed all of spring training with the shoulder issue. He resumed mound work about a week ago, but there's no timetable for his return to the active roster.

  • Royals' Carlos Hernandez: Light bullpen session this weekend

Hernandez (shoulder) is scheduled to throw a light bullpen session this weekend, Anne Rogers of MLB.com reports. Hernandez has been playing catch the last few days and will do so again Tuesday before progressing to some mound work shortly thereafter. The Royals have not ruled the reliever out for Opening Day yet, but it seems unlikely that he'll have time to ramp things back up by then.
